12/30/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Laylah upon arrival to the home setting. Upon arrival to the home setting, Laylah appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about her day. She discussed with QMHP-C that she was in a good mood and just preparing herself for school. Laylah discussed with QMHP-C that she would like to have an open session with QMHP-C about some personal issues. QMHP-C informed Laylah that QMHP-C would have to discuss with her mother the session if it included self-harm due to her being a minor. Laylah was understanding of QMHP-Cs’ simple request and what was mandatory of QMHP-C. 2. Behavior: Laylah contributed to the session and did not need any redirection throughout the session as she participated fully in session. Laylah was a little frustrated as she was discussing issues with her sexuality. 1. Intervention: QMHP-C began the session by inquiring about Laylah’s current thoughts, feelings and emotions. QMHP-C talked with Laylah about the characteristics of one of her female peers. QMHP-C inquired as to whether the peer has positive or negative quality characteristics for increasing positive peer relationships. QMHP-C assisted Laylah with processing her feelings and emotions. 2. Intervention: QMHP-C provided Laylah with role playing activities. QMHP-C conducted a review of the previous session. QMHP-C asked Laylah relevant questions to obtain Laylah’s level of comprehension. QMHP-C intervened by allowing Laylah to talk about her feelings as it related to the previous session about her father and how he does not do anything for her (he steals out in the community for her because he does not provide her like her mother does). 1. Response: Laylah appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Laylah mentioned negative comments about her peers and was redirected by QMHP-C several times. Laylah readily admitted this peer may not be the best peer and that she has been wanting to fight this peer for a long time to the point that she was going to meet her at her house and fight her because she doesn’t like her over a female that she likes and another peer is talking about it. Response: Laylah indicated to QMHP-C that she really likes this male and that this peer is going to have to back up. Laylah was respectful while QMHP-C was talking about the qualities of relationships. Laylah made positive and negative comments about her male/female peers. Laylah stated that she has more positive qualities than negative qualities.
